Most of the personal information we process is provided to us directly by you for one of the following reasons:

·      You have completed our “contact us” form on our website and have provided us with your name, email and phone number for the purpose of making an appointment or to discuss an enquiry with us.

·      You have contacted us via email to make an enquiry or appointment.

·      Via telephone contact.

·      Notes will be completed by us after each session which may include: any relevant medical information and the details of any treatment provided.

·      With your consent only, data may also be collected from other relevant health care professionals such as GP via phone, email or written documents.

We use the information that you have given us in order to provide safe and accurate physiotherapy assessment and treatment.

We may share this information with other relevant services and health care professionals such as GP’s consultants and NHS physiotherapists. Information will only be shared if necessary and relevant and only with your consent, which can be withdrawn at any time.

Under the UK General Data Protection Regulation (UK GDPR), the lawful bases we rely on for processing this information are:

Your information is securely stored in a range of locations. This includes digital format such as emails. Written documents are kept securely under lock and key.
